Create spaces for contact information. This might be the most important part of this type of form. You need to know how to get a hold of everyone in your league at all times. This is important for scheduling your draft, for letting people know about playoff schedules and structures and for contacting people if they are late setting their weekly roster. It is usually good to get several methods of contact on the form. Get a telephone number, an email address and a cell number where owners can be reached by text message.

If there is a fee for playing in your league, provide a space where owners can indicate whether payment is being submitted with the form. Some owners may choose to remit payment at a later date, so they should indicate on the form if they are paying immediately or later.

Ask owners to provide you with a team name. Most fantasy leagues operate under team names rather than owner names. You will need these names for either tracking results and scores on your own or for entering into the software or pool program that you use to manage your fantasy league.

List all the important dates relevant to your league on the form. A registration form isn't just a way to obtain information from those who are going to join your league; it is also a way for you to provide information to them. The dates that you want to provide, at the very least, are the draft date and the last days for registration and fees to be paid.

Include an availability form to fill out if you don't already have a date set for your draft. Give a few draft day options for your league, and ask owners to indicate which days and times might work best for them. This can help you decide the best time to hold your draft.
