Feature a head shot of yourself in uniform at the top of the bio sheet. Alternatively, feature a photograph of yourself in action on the soccer field.

State your hometown, high school and high school mascot. List all varsity letters earned in soccer or in other sports. State your parent’s names.

List any awards won at the high school level. List both personally earned awards and any team earned awards or trophies.

Ask a coach to write a recommendation on your behalf, if completing the bio sheet as a high school player for recruitment purposes. Attach the recommendation to your bio sheet.

Make a video of clips of yourself in action. Post the video online and provide a link to the video on the bio sheet.

List your major, or your intended major, if still in high school, and your GPA. List any and all academic awards, achievements or special recognitions, such as honor roll. Cite your career goals.

Profile your soccer achievements. Identify how many years you have played. List position(s) played, goals scored, and any tournaments or competitions in which you have competed. List any awards or scholarships earned for the sport.
