How do I Write a Soccer Referee Report?

Among the many duties of a soccer referee is to submit a referee report at the conclusion of a match. The objective of the referee report is to document and report relevant actions during the match for future reference. Important information in the report includes the teams involved in the match; the date of the match; the time of the match; the names of all participating referees; and all disciplinary actions, substitutions and goals. The referee must provide accurate information in the referee report and usually must turn in the report within 24 to 48 hours of the match's end, depending on the league requirements.

      Take out your referee report card (see Resources) before the match begins and record the name of the teams playing, both home and visitor. Note the date of the match and indicate whether you have checked the rosters of both teams. Fill in the names of all referees involved with the match.

      Record all the goals scored during the match for both the home team and the visitors. You must write down each goal as scored and note who scored the goal.

      Write down all substitutions in the soccer match as they occur. Make sure you note who is replacing whom and what position they play on the field. Note the name of the team, the player, the player's number and reason for substitution if one is given.

      Record all disciplinary actions when they occur and the name of the player, coach, or manager who received the disciplinary action. You will need to record all red cards and yellow cards handed out during the match and annotate why you handed them out. Also note the name of the team involved, the player and the player's number.

      Write down all injuries that occur during the match. Record the type of injury if known, the name of the player, and the player's number and team.
