Release of Liability

The personal training contract will act as a waiver of liability that will be signed by you and your client. There should be acknowledgment that personal training and exercise in general can be dangerous. It should also state that the client voluntarily assumes all risks pertaining to the training and releases you of liability for any resulting injury or illness.
Client Medical History

Your agreement should include a section where your client can include all pertinent medical history. They should inform you of all pertinent medical information as well as any previous exercise experience. This segment should be quite exhaustive. You can find examples of medical history forms online or you can order one from the American Council on Exercise.
Consent of Exercise Test

Before you begin training a new client, you should perform an exercise test to determine their fitness level. Your employment agreement should inform your client about the test and explain any details that your client will need. Because this may be the first time your client has exercised with a trainer, the agreement should also state that it is up to the client to stop the test and inform you if they feel dizzy, nauseous, short of breath or have any other unusual symptoms.
Pricing and Payment

Your employment contract should state in detail how much your client will pay for training sessions as well as when payment is expected. It should also state details about consequences for client lateness and cancellations as well as conditions for refunds.

You should include your contact information as well as that of your client. There should also be an emergency contact for your client.
Signature and Date

Keep A Copy of Your Personal Training Employment Agreement for Your Records At the bottom of the contract there should be a space for both you and your client to sign and date the document. You should make a copy of this document so that both you and your client have one to refer to.
